Transition Care

Transition care is short-term help at home after a hospital, rehab or skilled nursing stay. The first days and weeks home are when recovery is most fragile, and when an extra pair of hands does the most good.

Our caregivers can:

  • Prepare meals and help with bathing and dressing
  • Give medication reminders
  • Help with walking and transfers
  • Take your loved one to follow-up appointments
  • Keep up the exercises and care routine that home health set up

We work alongside home health agencies, whose therapists and nurses often show our team how to help safely with transfers, exercises or wound care. When the home health benefit ends, we can keep that care going. Our nurses can also reconcile the new medication list and talk with the doctor and pharmacy. If the move home is part of a bigger change, care management can coordinate the whole transition.

We can usually start the same day you call. A nurse is available 7 days a week to do the assessment and get care in place.
