Care management is professional help coordinating everything around an older adult’s care: doctors, medications, hospital stays, and plans for what comes next. At Home Careolina it’s nurse-led and overseen by Barbara Moore, a Certified Care Manager (CMC) and member of the Aging Life Care Association. It’s the part of our work that reaches beyond the home, and it can stand on its own or sit alongside home care.
We help with:
- Medication education and management, including organizing weekly pill boxes
- Nurse visits for higher-level care needs at home
- Chronic disease and complex medical needs
- Healthcare advocacy: finding new doctors, arranging referrals to specialists, and attending appointments
- Training and support for family caregivers, including long-distance families
- Long-term care planning: reviewing long-term care insurance policies and budgeting for care
- Preventing and managing a crisis
- Navigating hospital and short-term rehab stays, with referrals to palliative care and hospice when needed
- Moves to independent living or long-term care communities
When a client also has home care, our care manager works with the caregiving team, so the plan and the day-to-day care stay in step.
